Advance Care Planning is a process of reflection and communication with your family. It is a time for you to reflect on your values and wishes, and to let your family or caregivers know what kind of health and personal care you would want in the future if you were unable to speak for yourself.
It means having discussions with family and friends, especially your Substitute Decision Maker – this person will speak for you if you cannot speak for yourself. It can include writing down your wishes and talking with healthcare providers and financial or legal professionals. Over a three-month period, I will work with you through a powerful one-on-one self-compassionate healing process of your sacred end-of-life and LIFE discovery through Best Three Months. Advance Care and Healing Plan Best of Three Months B3M Includes: • End-of-Life advanced or immediate care planning with your family, patient and coordination with medical/hospice care providers. • Six-ten 1.5-hour meetings (approximately three to four meetings per month) in five domains of life. Grief coaching, healing and care planning sessions. Five Domains of Life: Physical (body) – Creating comfort, Safety, and healing environment. Emotional – Honoring feelings and relationships. Spirit – Honoring beliefs and practices. Mind (mental) – Legacy life review. Practical – Honoring the body after death (Funeral arrangements, Sacred Ceremonies and Celebrations. |
